» The cabinets in the 2-year-old area, 3-year-old area, 4-year-old area and school-age area were not locked and did not have a locking mechanism.

Details: The director will arrange for locking mechanisms to be installed at all bathroom cabinets used to store cleaning supplies. The staff will be instructed to store all cleaning supplies in locked areas. The staff will always use a locking mechanism at storage areas containing hazardous substances.

» There were not any disposable towels available as required for hand drying. Children and staff shared a cloth towel at each hand sink. Staff reported that the center used all the available disposable towels since Friday, July 8 in most bathroom and sink areas. The last of the disposable towels were used in the infant room the morning of July 12.

Details: The director will pick up an order of disposable towels from a new supplier this afternoon. The director will make sure a scheduled regular delivery of an adequate supply of paper towels occurs before running out of needed supplies. If a change of vendors occurs again, the director will have a backup plan for obtaining all needed center supplies.

» There was 4 inches of mulch, less than the required 6-inch depth, at the exit area of the tube slide of the composite structure. On the toddler playground, there was only 3 inches of wood mulch, less than the required six inch depth, at the fall zones of two spring cars.

Details: The director will arrange for the mulch to be raked and added to the fall zone areas of the two cars and the tube slide. The director will instruct staff to follow a regular schedule for maintenance of the resilient surfacing depth at fall zones of equipment. The staff will report any concerns about the need for additional mulch to the administrative staff.

» The staff person did not use the foot pedal to dispose of a wet diaper in the container and was seen lifting the lid by hand.

Details: The director will review the requirements for the diapering procedures with the staff person. The administrative staff will observe diapering procedures at different times to make sure the staff adhere to the required practices for safety and the prevention of the spread of germs and potential disease.
